Healing of diabetic ulcers with
Terrasil: improvement of the microcirculation. Product
evaluation study.
In 29 patients
with diabetic microangiopathy and plantar ulcers
we tested the efficacy of Terrasil® applied onto
the skin, around the ulceration for 4 weeks, to
study the healing rate associated with this
treatment. Color duplex scanning had been used
to exclude venous thrombosis, severe arterial
obstruction; Doppler had been used to evaluate
the presence of tibial pulses (to exclude
patients with severe ischemia and necrosis). The
ulcerations were a combination of:
a: reduced arterial pressure (above-necrosis
limits with average skin perfusion pressure >50
mmHg) .
b: diabetic microangiopathy
c: localised infection.
Terrasil was applied around and at the egde of
the ulcerated areas (max diameter ranging
between 2 cm and 1.1 cm) and on the ulceration
(after cleaning) three times daily. The cream
was applied after careful washing (2 minutes in
water at 40 C° with Amuchina) of the ulceration
and surrounding area. A neutral adsorbing paper
bandage - in contact with the skin was applied
under a skin protecting/saving foam layer. An
adhesive bandage (Tensoplast) – or, as an
alternative - elastic stocking (Sanagens,
Boston), including the ulcerated area were used
during the observation period.
Results. Treatment with Tetrasil was more
effective than the management used in controls.
In treatment patients skin PO2 was increased
(28%), LDF (abnormally increased around the
ulcerated areas) was decreased (median 29%).
Flux increase is generally associated with
severe microangiopathy. Also the venoarteriolar
response of the area was significantly reduced
(<30%) at inclusion and improved at the end of
the four weeks in the treatment group (+16%).
The ulcer areas were significantly smaller at 4
weeks (max diameter range was between 0.23 cm
and 0; p<0.05) in the Terrasil group with
complete closure in 39% of subjects (vs 16% in
controls) (p<0.05). In conclusion, in diabetic
ulcerations, local treatment with Terrasil
improves the microcirculation and healing rate.
